1) Fill out the enclosed medical forms as accurately as possible. One section is used for each member of your family. (The preferred hospital section is just a preference, the ambulance crew reserves the right to transport the patient to a facility based on the patient's condition.) If you would like to completed your medical forms online, you can setup an account by going to http://www.vialoflife.com, and print our your information to be placed in the vial.

2) Fold the paper in half, and rold it inside the vial that is given

3) Please secure the vial to the tope right compartment of your refrigerator door with duct tape.

4) Please the sticker given on the outside of your front door, so that if the Watchung Rescue Squad has to respond to your house, they know that you are a participant of the Vial Of Life program.

Make sure to update your vial if you change your medicines or any new medical conditions develop.
